Video 7.7. Internal Rotation Lag Sign
Full description
The examiner holds the patient’s right arm in internal rotation behind the patient’s back with the dorsum of the hand facing the back and the shoulder in about 20 degrees of extension. The examiner supports the patient’s elbow as well as the wrist. Then, the examiner releases the wrist, and the patient is not able to maintain the position. The patient’s hand falls against their own back. This positive sign can indicate a tear of the subscapularis tendon. The examiner tests the contralateral side, which is negative.
