SLAP tears are common in the high-level throwing athlete. Here, the examiner performs O’Brien’s active compression test on a collegiate baseball player for the assessment of a potential SLAP tear. The patient here has a positive active compression test, demonstrating pain with the thumb pointing downward but reduced pain with the thumb pointing upward.
